New Patient Information

If you are a new patient with us at Grand Junction Endocrinology, you must arrive 20 minutes before your scheduled appointment to fill out necessary forms in our office.

You may also digitally fill out or print the forms below and fill them out in advance. Once completed, you may email them to us at gjendo@gjendo.com or bring them in at the time of your appointment.

After you have booked your appointment, you will receive a portal invite via email. The portal can be used to access your patient records.

Once you have created your portal account, we will send you a unique verification code to finalize your account setup.
